Job Summary: This position is located at the VA Medical Center, Homeless Veterans Outreach Clinic in Spokane Washington. Incumbent functions as the Tribal Case Manager for Mann-Grandstaff VAMC's Housing and Urban Development vouchers assigned to local tribes. The incumbent is responsible for program oversight, monitoring, and administration. The Tribal HUD-VASH Program falls within the Homeless Programs section of the Behavioral Health Services Care Line located in Spokane, Washington.

Major Duties:

Program Administration and Organization Implements and maintains referral and screening procedures for potential Homeless Program Veteran participants that meet national policy as well as the needs of medical center behavioral health services. Works closely with the VA medical center's homeless programs, residential and community care programs. Works independently in a rural and remote environment as the "systems expert", providing intensive case management services that focus on areas of readjustment and re-entry counseling, housing, mental health and psychosocial needs, barriers to treatment and mitigating risk factors related to incarceration and readjustment back to the local community. Veterans eligible for Homeless Programs come from a wide geographic area. Homeless Programs requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal, and tribal agencies. Clinical Assessment Interviews the Veteran and/or their family members to establish facts about the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veteran's functioning and health. Evaluates each Veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion. Comprehensive Case Management Provides case management for a complex caseload of chronically homeless Veterans (who must also be an enrolled member of a federally recognized Tribe) in the HUD-VASH programs in rural and remote regions of Eastern Washington, often on reservations. This role differs from traditional HUD-VASH case management in that Veteran family cases are complex, involve multiple causes of impairment at different levels, multiple systems in the community and require extensive knowledge of and collaboration with tribal resources. Develops psychosocial treatment plans, actively involving the Veteran and/or their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment. Refers Veterans to appropriate VA and other government/community services. Understands disparate values and reaches Veterans and their families from different social and ethnic groups while communicating confidence in the Veteran's ability and worth, helping the Veteran address problems utilizing larger support systems as appropriate. Implements the Housing First model within the Homeless Program. Generates the initial documentation to tracks homeless Veterans and services they receive while assigned to care. Explains Veteran's treatment and progress to third parties as appropriate and manages the quality control and evaluation of the professional services provided. Provides psychosocial services and is accountable for the overall effectiveness of those services. Modifies services to best meet the treatment needs of Veterans and to promote efficient practice and coordinates with other services offered in the treatment program. Outreach Provides direct outreach services to areas where people experiencing homelessness congregate; including the street, remote campsites, and homeless camps under the freeway, most of which are often not suitable for human habitation. Qualifications: To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement. Basic Requirements: Citizenship. Be a citizen of the United States. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Experience working with Native American Tribes and working with people experiencing homelessness. Physical Requirements: Day-to-day work involves sitting, walking, standing and light and moderate carrying and lifting objects. The position requires the ability to drive a government vehicle and travel in the service area and local community to perform duties, with some overnight travel to remote parts of the catchment area required. The work requires the use of a computer. Work may require assisting physically disabled Veterans. The work typically occurs at an office-based workstation in the Homeless Program Outreach Clinic in downtown Spokane, with adequate lighting and heat. Work is also conducted in the community, including but not limited to various shelters, soup kitchens, churches and public and private agencies where the working conditions are not overseen by the VA.
