Role Summary

The Clinical Consultant / Case Manager – Health Advocacy supports the delivery of integrated health advocacy services. This role partners with clients, internal teams, and associates to promote engagement, improve health outcomes, and support population health strategies through clinical expertise and data-driven insights.

Primary Responsibilities

  • Serve as a clinical subject matter expert supporting health advocacy programs and clinical operations.

  • Act as a liaison between associates, client Human Resources teams, and internal partners.

  • Communicate and educate stakeholders on clinical programs, services, and updates.

  • Analyze engagement and utilization data to identify population health trends and opportunities.

  • Conduct regular reviews of engagement metrics and outreach outcomes.

  • Partner with informatics teams to support dashboards and reporting.

  • Support participation in case management, coaching, and specialty care programs.

  • Document outreach activities, customer interactions, and outcomes.

  • Support client engagement activities and worksite events as needed.

Health Equity and Social Determinants of Health

  • Analyze social determinants of health data to identify risks and opportunities.

  • Collaborate with clients and internal partners to support diverse populations and reduce health disparities.

Education and Continuous Improvement

  • Maintain clinical expertise and stay current on health promotion and wellness strategies.

  • Educate client populations on available resources and clinical programs.

  • Contribute to continuous improvement through planning, analysis, and feedback.

Qualifications

  • Active Registered Nurse (RN) license in state of residence

  • Minimum of three (3) years of clinical practice experience.

  • Experience in nurse case management or health advocacy preferred.

  • Strong analytical, reporting, and data interpretation skills.

  • Effective verbal and written communication skills, including presentation experience.

  • Consultative mindset with strong problem-solving and collaboration skills.

  • Ability to travel up to 50% as required.
