Job Summary: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Primary responsibilities fall within the HCHV team portion of homeless programming including: community outreach, clinical assessment and services to HCHV clients, daily homeless program coverage including oversite of coverage calendar, initial triage of coverage clients, ongoing case management of coverage clients, monitoring and facilitation of HCHV contracts including ANR and CRS contracts, knowledge of and episodic back-up assistance with GPD and CES programs, shared oversite of Deshon Place program (with HUD-VASH Coordinator and GPD Liaison), assisting HCHV Supervisor with monitoring data related to homeless programming. Major Duties include but are not limited to: Responsible for the identification of homeless veterans and the provision of VA HCHV services to veterans who are homeless or are vulnerable to homelessness through community outreach Responsible for the provision of effective case management services designed to resolve the problem of homelessness among our Nation's veterans to provide a continuum of health care and support services through various programs within VA and joint collaborative efforts with non-VA providers of assistance to homeless people Responsible for the provision of VA HCHV services in areas known to and frequented by veterans who may be homeless; such as soup kitchens, missions, shelters, jails, day labor stations, and campsites Responsible to provide the total psychosocial assessment needs for the VA Butler Healthcare Homeless Veterans Program Offer expert opinion to other mental health professionals and allied health personnel on the basis of observation, interview, and evaluation of patients using therapeutic procedures in which he/she has received special and extensive training and brought to a high level of skilled usage Consider mental status and psychosocial functioning of the patient as it relates to ability to understand directions, provide objective self-assessment, or any other medical milieu appropriate to the overall treatment plan Capable of practicing at the advanced level and has specialized knowledge of practice typically related to a particular diagnosis or population Administers information and analytical systems to evaluate and enhance the quality of service provided to patients Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information Uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements Performs program analysis, development, review, design, and contact work as possible with various agencies (public, DOD, state, etc.) Performs work related to the review, monitoring, analysis, planning and assessment of various programs Responsible for key project activities related to the planning, development, and implementation of the project component. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer Practices independently, using professional judgment and expertise to make decisions about services and treatment provided to veterans and their significant others/family members Effectively communicates with other disciplines to facilitate treatment planning and implementation Participates effectively in team meetings, treatment planning conferences Collaborates with divergent multidisciplinary team members in a manner that enhances coordination of comprehensive patient care Meets the needs of customers both internally and externally while supporting the VA mission Performs other related duties as assigned. Qualifications: Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments.
