Experience a place where your discipline is an important contributor to patient experience. In this role, you will support families of children undergoing new tracheostomy placement by facilitating program enrollment, providing psychosocial support, and coordinating post-discharge follow-up. This role will involve active participation in interdisciplinary rounds, family engagement prior to and preparation for discharge, structured follow-up after hospitalization, and collaboration with clinical and grant teams to ensure high enrollment and program fidelity. You will also serve as a valuable resource as you communicate with patients, their families and the highly skilled interdisciplinary team to assure excellent outcomes and an outstanding patient experience.

This position is part-time.
Preferred Criteria
At least one year of inpatient pediatric experience.
Required Criteria
Master’s Degree in Social Work (MSW) New York State License as a Licensed Masters Social Worker (LMSW)
Salary Range:
