About the Program
The GEM@Home and Rehab@Home service is a bed substitution model, replicating bed-based services in the patient’s home where appropriate. The model of care enables an earlier supported discharge for SVHM. Patients with conditions associated with rehabilitation needs, ageing, cognitive dysfunction and chronic illness are prioritised for referral. The primary aim of the service is to assist patients in achieving their goals of care in their own home. The service is interdisciplinary and operates 7 days a week with access to professional support and supervision coordinated by the SVHM Social Work Department
About the Role
The Social Worker is responsible for the development, implementation and delivery of a high-quality social work service to patients and families across GEM@Home and Rehab@Home. Working within a flexible, interdisciplinary structure, you will be expected to manage a caseload, incorporating the proficient use of your clinical skills, evidence-based practice, effective communication and collaboration.
Additionally, education, research and supervisory responsibilities are undertaken as are evaluation and quality improvement activities.
Provide optimal clinical interventions for patients in need of social work. This includes effective psychosocial assessment, risk assessment, goal setting, intervention, liaison and discharge planning
Communicate assessment and treatment outcomes to nursing, medical and other Allied Health staff via documentation and relevant meetings
Provision of discipline specific knowledge, skills, consultation and resources
Supervision of staff and students as required
Participation in quality improvement activities
Classifications for this position will range from SC21 - SC24 ($90,261 - $103,724 per annum, pro rata for part time).
Your Contribution
Bachelor of Social Work Degree/ Qualifying Masters Degree
Eligible for membership of the Australian Association of Social Work (AASW)
Current Victorian Drivers License
Advanced social work skills as evidenced by four or more years post- qualifying professional experience in an inpatient hospital setting
Demonstrated knowledge of the application of social work theory, practice frameworks and principles, in the context of the current health care environment
Experience with NDIS applications and My Aged Care
Current National criminal history check, or willing to obtain
