Social Work Assistant – Inpatient - Days
Social Work Assistants' play an intricate role in assisting with patient flow and ensuring quality care and delivery of services for our patients. You’ll partner closely with an interdisciplinary team, working in tandem with the Social Worker and Care Coordination team. Assist in arranging transportation for patients, update and manage documentation within electronic medical records systems. Connect with insurance companies, obtain authorizations and act as liaison with community resources and Nursing Homes. Attend rounds with the physicians and care team and mobilize community agencies and other support networks to give assistance.
This position is 5 days a week, Mon – Fri, 8:30 - 4:30pm.
Preferred Criteria
Bachelors Degree in Social Work
Case Management experience
Experience working with EMR systems
Desire to collaborate and exceed expectations in providing excellence customer service and patient care
Strong organizational skills and communication skills
Bilingual Spanish
Required Criteria
Bachelors Degree
Work experience in a hospital social work department and/or agency
Proficiency in documentation within electronic information systems
Salary Range:
