Registered Nurse (RN) – Inpatient Care Manager

Chicago, IL (Lakeview Neighborhood) 

Full time; Mon-Fri; 10 hour shifts

Where You Will Work: Part of the care management services team focusing on patients with the complex medical and/or psychosocial needs.

This position is a float Care Manager.

We’re Looking For:

  • Completion of an accredited or approved program in nursing
  • Bachelor’s degree in nursing 
  • Registered Nurse license issued by the state of Illinois
  • 2 years of clinical nursing experience and 3 years of care management experience
  • Accredited Care Manager (ACM) certification issued by the American Case Management Association (ACMA) needs to be obtained within 2 years, or 
  • Care Manager Certified (CMC) certification issued by the National Academy of Certified Care Managers (NACCM) needs to be obtained within 2 years, or 
  • Nurse Case Management (RN-BC) certification issued by the American Nurses Credentialing Center (ANCC) needs to be obtained within two (2) years. 

What You’ll Do:

This discharge planning specialist carries a caseload of complex patients identified by length of stay, dollar, or time intensive discharge planning outlier designation and serves as a resource for staff. Analyzes data to understand the causes and corrective actions for this subset of patients. Effective utilization of organizational resources to assist the medical center in achieving its goals of high-quality, cost-effect patient care and service. Collaboration with community-based resources and Payor continuity of care across the continuum. Functioning in a manner that supports compliance with regulatory standards and agency requirements.
