Position Purpose
At Saint Francis Hospital, the Emergency Department Registered Nurse Case Manager plays a critical role in coordinating patient care in a fast‑paced emergency setting. This role focuses on early identification of patient needs, care coordination, utilization management, and safe transitions of care to ensure high‑quality, cost‑effective outcomes while supporting patient flow and throughput from the Emergency Department.
What You Will Do
As an Emergency Department RN Case Manager, you will serve as a key partner to ED physicians, nurses, and interdisciplinary teams to support timely, appropriate care decisions. Responsibilities include:
Collaborating with ED providers and care teams to assess patient needs upon presentation
Identifying appropriate levels of care and facilitating admissions, transfers, or safe discharges
Coordinating care across inpatient, observation, and post‑acute settings
Supporting patient flow, throughput, and length‑of‑stay initiatives
Conducting clinical, psychosocial, and discharge planning assessments
Applying nationally recognized medical necessity and utilization criteria
Ensuring compliance with regulatory, accreditation, and payer requirements
Educating patients and families on care plans, resources, and next steps
Advocating for patients while balancing quality, safety, and resource utilization
Required:
Graduation from an accredited nursing school
Valid RN License in Connecticut
Preferred:
Bachelor’s degree in Nursing
Minimum of three (3) years of acute care RN experience required
Emergency Department experience strongly preferred
Demonstrated knowledge of utilization management, level‑of‑care determination, and care coordination
Working understanding of CMS/HCFA, OBRA, State of Connecticut regulations, and Joint Commission (JCAHO) standards
Certification:
BLS certification required
Case Management certification (CCM or ACM) preferred or obtained within a designated timeframe
