Job Description Summary
The Social Services Assistant reports to the Social Work Manager of the Institute of Psychiatry. Under clinical practice guidance of the Social Work Manager and Licensed Social Worker on the assigned unit, the Social Services Assistant will assist with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role will provide support to the Licensed Social Worker on the assigned unit in way of collateral calls, scheduling outpatient follow-up appointments and assisting with required documentation to facilitate transitions to appropriate level of care.Entity

Job Description

Part-time position - 2 days a week on the Senior Care Unit at the Institute of Psychiatry
Additional Job Description
Education: Bachelor's degree in a social work or other appropriate social services program (psychology, sociology, etc. ).
Credentials: If holding a Bachelor's Degree in Social Work, must have a BSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
