Our mission is to develop strong pathways into drug and alcohol treatment for individuals impacted by homelessness, mental health challenges, domestic abuse, and experiences of the Criminal Justice system.

About the Role

As a Prison Pathways Coordinator, you will play a vital role in ensuring a smooth and supportive transition for individuals moving from custody into the community. • Maintain accurate records and compile performance data and reports

  • Work collaboratively with HM Prison Service, probation teams, commissioners, and other external stakeholders

  • Promote and facilitate referrals into substance misuse and wider support services

  • Use internal systems and Microsoft Office applications to manage caseloads and communications

  • Contribute to continuous improvement of criminal justice pathway processes

To Succeed in This Role, You’ll Need:

  • Knowledge and understanding of the Criminal Justice sector

  • Experience or awareness of substance misuse and recovery work

  • Strong organisational and communication skills

  • Ability to build positive partnerships with a range of agencies
