The PGY1 pharmacist residency is a structured, 1-year postgraduate educational and training experience that fosters the development of advanced knowledge and skill in the delivery of pharmaceutical services within a community hospital setting.Major Responsibilities:
The following key areas (Clinical, Operations/Distribution, Financial, Safety and Regulatory, Research and Education) are performed by the Pharmacy Resident (PGY1) through the completion of rotations under the supervision of preceptors in each specialty which includes, but is not limited to, anticoagulation, antimicrobial stewardship, critical care, family medicine, internal medicine, oncology, medication safety, practice management, transitions of care, and staffing and operations.
Clinical Performance:
Provides medication management services for all assigned patients in a collaborative manner that consist of the following:
a) Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop/modify therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication related issues in electronic health record, interpret laboratory data, review patient medication profiles, monitor pharmacokinetics of drug therapy, monitoring for potential adverse drug – drug, drug-lab, drug-disease state, and drug - food interactions.
