SUMMARY

            

LOCATION:  ElderONE      

DEPARTMENT: EO ADM CLINIC

SCHEDULE:  7:30am - 4:00 pm OR 8:00am - 4:30 pm   

ATTRIBUTES

  • Reliable transportation to meet participants at scheduled appointments

  • Experience with elderly population preferred

  • Experience with direct patient care preferred

  • HHA, CNA, PCA preferred

RESPONSIBILITIES

  • Complete activities as listed within the participant care plan as required during each encounter. 

  • Meets participants at scheduled appointments.

  • Assists participants with checking in and checking out for appointments.

  • Provides assistance with needs while attending the appointment including (but not limited to): supervision, transfers, dressing and undressing, toileting, assist with positioning.

  • Communicates with Appointment Coordination team and Transportation to review any cancellations, no shows, change in appointment times.

  • The Participant Escort will collaborate with Rochester Regional Home Care and ElderONE Appointment Coordination Team to ensure care plan needs and appointment times are provided timely and efficiently.

  • The Participant Escort will provide care within ElderONE clinics and assist within the appointment coordination team as appropriately outlined in required responsibilities for care needs. This may occur when appointments are at lower than normal volumes, or participants appointments are canceled.  

  • Communicates with ElderONE clinic for medical and/or clinical needs that may arise during the appointment.

  • Performs other duties as assigned.

EDUCATION:

LICENSES / CERTIFICATIONS: 

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
