Nurse Practitioner- Hospice, Per diem

 

BAYADA Hospice is hiring a compassionate and dedicated Per diem Nurse Practitioner, (NP) to join our team.  

Prior home care, hospice, or palliative care experience strongly preferred, but not required. As a hospice nurse NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Nurse Practitioner (NP) Responsibilities:

• Making home face to face hospice recertification visits to clients in designated geographic territories in NH.
• Available for on call one weekend per month from Friday 5pm to Monday 8:30am to support clinicians in the field telephonically for symptom management and proscription refills.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.

Qualifications for a Nurse Practitioner (NP):

• A current license as a NP in VT and NH.
• Active DEA in both VT and NH
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care, hospice and/or palliative care experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
• Strong interpersonal skills.

 

Base Pay, depending upon qualifications and subject to negotiation:

$60-$70 per hour
