As a Nurse Practice Manager, you oversee the end-to-end intake and coordination of care for surgical candidates, supporting one to two surgeons and managing all patients referred to their practices for surgical evaluation and treatment. You lead a highly coordinated intake process by gathering essential clinical and functional health information in advance, enabling efficient appointment scheduling, streamlined workflows, and reduced patient wait times. This role ensures that each surgeon’s practice operates smoothly while maintaining compliance with regulatory standards and The Joint Commission requirements.
Coordination and continuity of care are central to the role. You collaborate closely with primary care providers, referring physicians, cardiology teams, surgeons, fellow nurse practice managers, and other healthcare professionals to address both pre- and post-surgical needs. Responsibilities include reviewing clinic information with surgeons, scheduling appropriate tests and appointments, directing additional studies to outpatient scheduling as needed, and facilitating hospital transfers in partnership with scheduling officers and surgeons’ offices—ensuring a seamless, patient-centered surgical experience from referral through recovery.
A caregiver in this position works days (4 x 10s) 7:00am—5:30pm.
A caregiver who excels in this role will:
Manage the intake process for surgical candidates by obtaining clinical information relative to the patient state of health and level of functioning.
Ensure departmental compliance with regulatory and The Joint Commission requirements.
Coordinate with the PCPs, referring physicians, patients, CCF Cardiology staff, surgeons, office staff, other NPMs, scheduling officers and preop/TCI/postop Nurse Clinicians to assure continuity of care and appropriate concern for individualized presurgical and/or postsurgical needs.
Review all clinic information available for review with surgeon for appropriate scheduling and communicate additional studies to the outpatient scheduling office under physician direction.
Facilitate hospital transfers in collaboration with scheduling officers, access plus and surgeon office.
Minimum qualifications for the ideal future caregiver include:
Bachelor’s of Science in Nursing (BSN) from an accredited school of Professional Nursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
Four years of experience as an RN in Medical Cardiology or Cardiac Surgery clinical areas
Physical Requirements:
Requires the ability to walk to various locations throughout the foundation, normal or corrected vision and hearing to normal range and the ability to work and function in a stressful environment.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
