A Brief Overview:
The purpose of the Social Woker position is to provide various social services to Symptom Support Program and Palliative Care patients and families including but not limited to psychosocial evaluations and assessments, ongoing counseling, direct casework services, bereavement services and referrals to community resources as ordered by the physician and/or APN according to the Palliative Care and Symptom Support Program process. This position reports to the Practice Manager of Palliative Care.

  • Ongoing growth and development from participation in events such as workshops, in-service programs and departmental meetings.
  • Provide service based on physical, psychological, educational and related criteria, appropriate to the age and type of the patients/customers served in their area.
  • Provide and document assessments, psychosocial evaluations, and on-going counseling to patients (in the patient's residence, nursing facility, or at Endeavor Health offices) referred to Palliative Care and the Symptom Support Program.
  • Along with the patient, family, and interdisciplinary team develop appropriate plan of care.
  • Patient advocacy including but not limited to education of patient and family rights, case management services, and staff education regarding identification and reporting of abuse/neglect.
  • Assist in transition to other levels of care/services.
  • Completes and adheres to all credentialing/privileging requirement and guidelines, peer reviews, quality assurance guidelines and all practice & program directives.
  • Collaborates with administrative team to create and maintain scheduling template, follows billing and documentation guidelines, and participates in relevant training.
  • Participates as a preceptor with new orients and students as needed.
  • Participates in committees and projects as requested.
  • Functions as a resource in the community and networks with other agencies and groups representing Endeavor Health Palliative Care.
  • Participates in Palliative Care/Symptom Support Program staff and team meetings.
  • Participates in continuing education programs per license requirements.
  • Participates in Palliative Care performance improvement activities.

What you will need:
  • Masters Degree Social Work Required
  • 2 Years of post-graduate experience in the field of social work
  • Must possess a high degree of interpersonal skill not only in relationship to individual patients and their families, but the interdisciplinary staff as well.
  • Must be firmly rooted in professional knowledge of social work, as well as have the flexibility to work with the professionals of other disciplines in order to carry out these functions effectively.
  • Must possess developed clinical skills and intervention strategies for work with high-risk population.
  • Knowledge related to adjustment to health and disability, death and dying essential.
  • Ability to work independently important.
  • Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR) Required Or
  • Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR) Required And
  • BLS – Basic Life Support (CPR and AED) - American Heart Association (AHA) Required
