What You'll Do 

The Manager is responsible for oversight of the day-to-day operations and performance of the Enterprise RN, Intake Team. This individual is accountable for training, auditing, providing team support, and leveraging program data to deliver outcomes. The Manager will lead efforts to develop, implement, evaluate, and iterate intake and after-hours nursing practice standards, and will work closely with regional clinical leadership  to assist patients safely, effectively, and efficiently across the continuum of care. The role also provides oversight of Express Lane support, market expansion coverage, and patient engagement efforts. This role reports directly to the VP, Clinical Services. 

Essential Functions 

  • Develops and leverages reporting tools to enhance visibility of program performance and outcomes, and to assess the effectiveness of case management efforts. 
  • Partners cross-functionally to address issues related to intake and after-hours tracking, trending, and documentation. Collaborates closely with VP, Patient Growth on workflow challenges and trends. 
  • Oversees daily operations of the Intake team, ensuring timely and accurate completion of patient initial assessments and smooth transition into intensive or longitudinal care within the regional clinical teams. 
  • Oversees daily operations of individuals working the late shift Registered Nurse – Intake schedule from 10am – 7pm local time. 
  • Provides leadership oversight for Express Lane operations, ensuring HRAs, care plans, and follow-up scheduling are completed accurately and timely. 
  • Monitors staffing and coverage to ensure patients are seen by an Intake RN in a timely manner. 
  • Supports market expansion and coverage by deploying resources to assist with new launches, market growth, and temporary staffing gaps.  
  • Models expert clinical practice, leadership, and critical thinking skills to efficiently coordinate and direct patient care while maximizing use of resources. 
  • Conducts chart audits per established policy to ensure documentation standards and compliance requirements (NCQA, CCM) are met. 
  • Assists with recruiting support, and completion of employee performance reviews. 
  • Communicates openly with the Strive’s Enterprise Clinical Leadership and staff to share program updates and patient successes. 
  • Participates in Strive leadership development opportunities. 
  • Coordinates coverage for both the Intake and After-Hours teams, including PTO requests, task and queue management, and on-call coverage. 
  • Meets in person with internal and/or external stakeholders to facilitate team and business priorities/opportunities. 

 

Minimum Qualifications 

  • Associate’s degree in Nursing, Healthcare Business, or related field.  
  • 6+ years combined of related education, experience, or certification. 
  • Active licensure as a RN, Physician Assistant, Nurse Practitioner, or other clinically related licensure. 
  • Ability to travel and be onsite to meet business needs. 
  • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms. 

 

Preferred Qualifications 

  • 1+ year experience being in a people management position. 
  • Strong analytical skills. 
  • Experience working with CKD/ESRD. 
  • Strong project management skills. 
  • Demonstrated leadership capabilities.
  • Expertise in interpersonal relationships, organizational relations, and effective communication, negotiation, and conflict resolution skills within a matrixed environment. 
  • Value-Based Case and payor experience.  
  • Discipline to work independently with minimal supervision.  

 

Annual Base Salary Range: $99,000 - $124,000
