Job Description:

Essential Duties and Responsibilities:

Assist community coalitions in developing and advertising KY MOMS MATR program and participation. Deliver prevention services to universal, selective and indicated pregnant and postpartum women. Deliver SBIRT (Screening Brief Intervention, Referral to Treatment) trainings to potential referral sources. Develop robust, effective and collaborative referral protocols with multiple community partners. Organize communities to provide prevention trainings to individuals, agencies, or groups within the community. Participate in state initiatives as directed by Department for Behavioral Health and complete all necessary requirements. Represent company throughout various boards and committee meetings, within the seven counties. Inform supervisor, and other members of the team as indicated, of issues/concerns related to individual clients. Participate in continuing quality improvement activities Create and maintain awareness of prevention efforts through various marketing strategies. Gain additional trainings and certifications to maintain and enhance this position. Maintain confidential client files and bill for prevention services in AVATAR system under a clinical billing supervisor. Participate in ordering materials and incentives for participants. Work with a team to complete the annual KMM grant application and ensure grant deliverables are met.
Maintain accurate, thorough records on all assigned cases per board policies and procedures. Document all activities in the Prevention Database System. Ensure the accuracy, content and completeness of paperwork. Provide services within time requirement as set forth in policy and procedures. Collect, create, organize and maintain resources accessible to the community.

Job Qualifications:

  • Bachelor’s degree in Social Work, Psychology or, Health Education or related field.
  • Knowledge of substance use disorders, community supports and ability to work well in a team environment.
  • Experience with public speaking and presentations.
  • Familiar with culture, demographics and languages of the community where the program will be implemented.
  • Possession of a valid driver’s license and access to reliable means of transportation.  Some day and overnight travel required.
  • Must obtain a Certified Prevention Specialist (CPS) from 3 years of date of hire

Required Competencies, Skills, and Knowledge:

  • Strong verbal and written communication including facilitation and public speaking. 
  • Proven ability to build relationships, trust and deepen connections with diverse people.
  • Knowledge of positive youth development strategies.
  • Ability to follow through on assignments with minimal oversight and manage multiple tasks simultaneously.
  • Knowledge of Microsoft products (Word, Excel, PowerPoint).
  • An active driver's license is required
  • Good driving record
  • Reliable transportation is required for this position

Amount of Travel/Other Special Conditions or Requirements

Region-based; regional and statewide travel is required.
