What You’ll Do

In this role, you will support hospice patients and collaborate closely with RN Case Managers by:

  • Carrying out individualized plans of care with precision and compassion

  • Providing skilled nursing interventions to promote comfort and symptom management

  • Offering education and emotional support to patients and families

  • Observing changes in condition and promptly reporting updates to the care team

  • Helping patients remain safe and comfortable in their homes, reducing avoidable hospital visits

  • Building trust through consistent, thoughtful communication

What You’ll Bring

  • High School Diploma or GED

  • Graduate of an accredited LPN/LVN program

  • Current LPN/LVN license in the state of practice

  • At least 1 year of clinical experience (hospice or home health a plus)

  • Reliable transportation, valid driver’s license, and insurance

  • Ability to lift 50–100 lbs and assist patients safely when needed

Compensation

  • Hourly range: $25.00–$37.00
    (based on experience, education, skills, and internal equity)

  • Additional earning potential may include on-call pay, performance bonuses, or other incentives
