Registered Nurse (RN) – Home Health | Fort Wayne, IN
You became a nurse to care for people, not rush through a shift.
At Elara Caring, you provide one-on-one care in the patient’s home. You manage your day, build real relationships, and see the impact of your work in real time.

What you’ll do:
✔ Manage your own patient caseload
✔ Complete start of care and follow-up visits
✔ Develop and update care plans
✔ Communicate with physicians and care team
✔ Document visits accurately (HomeCare HomeBase)
✔ Monitor patient progress and adjust care as needed
What you need:
✔ Active RN license (Indiana)
✔ 1+ year clinical nursing experience
✔ Valid driver’s license and reliable vehicle
✔ Ability to travel within Fort Wayne and surrounding areas
✔ Home Health or Hospice experience preferred
