The Clinical Pharmacist – Outreach Quality and Affordability is responsible for developing individual and population-based solutions to improve HEDIS performance, support affordability , partner on complex case management, and conduct comprehensive and targeted medication management in selected members to identify, resolve, and prevent medication-related problems. The Clinical Pharmacist documents the care delivered and communicates essential information in PBC’s documentation system, along with providing  the member’s health care providers and the health care team. The Clinical Pharmacist is also focused on achieving optimal use of medications, emphasizing dosing, monitoring, identification of adverse effects, and economic efficiency to achieve optimal member outcomes. This position will collaborate closely with case management, quality, medical management and pharmacy and client consulting  teams.

What you will do:

  • Utilize prescription and medical claim data to generate clinical recommendations for medication management and retrospective Drug Utilization Review (DUR)
  • Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
  • Assist case managers in identifying and eliminating barriers to care.
  • Provide telephonic consultation and as required, visits to provider clinics and member homes.
  • Help design drug therapy components of disease management algorithms.
  • Utilize health plan’s formulary information to guide appropriate medication recommendations
  • Knowledgeable of current and emerging clinical, operational, and management trends
  • Active participation in various clinical departmental meetings and forums to improve clinical programs, enhance processes, and share clinical information
  • Continuously look for ways to streamline and improve workflow
  • Assist with user acceptance testing for new clinical programs and system enhancements.
  • Assist Pharmacy leadership in analyzing and researching pharmacy cost trends and clinical program opportunities.
  • Support and serve as a preceptor for pharmacy students and/or residents, introducing them to drug utilization management and care management activities.
  • Perform other duties as assigned.

What you will bring:

  • Pharmacist Degree (Required)
  • Active WA State Pharmacist license (Required)
  • Clinical pharmacy training, clinical residency training preferred from an accredited PGY-1 Residency program (Required)
  • Two (2) years’ experience as a pharmacist (Required, 3 years preferred)

Strong Preference given to candidates with:

  • Ambulatory and clinic base care experience
  • Completion of PGY-1 residency
  • POP Health experience
  • Clinical program development
  • Motivational Interviewing Skills

The pay for this role will vary based on a range of factors including, but not limited to, a candidate’s geographic location, market conditions, and specific skills and experience.

The salary range for this role is posted below; we generally target up to and around the midpoint of the range.

National Plus Salary Range:

$123,200.00 - $209,400.00

*National Plus salary range is used in higher cost of labor markets including Western Washington and Alaska.
