Overview:
This position serves as a trusted liaison between Sentara Healthcare and pregnant and postpartum individuals within the community. The role integrates community health outreach with culturally responsive perinatal services to improve maternal and infant health outcomes, reduce disparities, and address social determinants of health. Responsibilities include providing education, advocacy, emotional support, care coordination, and system navigation throughout pregnancy, childbirth, and the postpartum period.
Bilingual candidates are encouraged to apply; proficiency in another language is preferred but not required.
This role works with a variety of grant funded programs to bridge the gap between organizations, including health care delivery systems and community health services to: increase culturally relevant knowledge about health and wellbeing; improve underserved populations’ access to services; connecting patients to community resources; and ultimately improving health outcomes.
Education
High School Diploma or Equivalent (Required)
Certification/Licensure
Valid Driver's License with an acceptable driving record (Required)
Experience
One year of community health worker experience (preferred)
