Summary:
The Community Health Worker will support the Mobile Farmer’s Market. This position is hands-on and community focused - delivering fresh food directly to neighborhoods. The ideal candidate is comfortable driving, adaptable, kind, comfortable engaging with members of the community, actively involved in moving inventory, and working outdoors in all types of weather. They should be ready to roll up their sleeves and work closely with the current mobile unit collaborative team.
The Community Health Worker (CHW) strives to improve access to care and patient health by providing outreach that aides in alleviating barriers through advocacy, resource navigation, and connections to social supports. The CHW focuses on bridging the gap between diverse communities and Virtua Health by fostering relationships, developing trust, and providing resources to improve the overall wellbeing of the community.
Position Responsibilities:
Develop relationships, conduct preventative health screenings, provide health-related education and resource information to members of the community.
Educate and/or enroll patients and/or community members to needed programs and services, track appointments, and provide follow-up for missed appointments and opportunities, complete data as needed.
General clerical duties, inventory/order of supplies, attend resource events and display set up
Position Qualifications Required:
Required Experience:
Demonstrated interest in community, social justice, public health and/or local economies. Must display ease and comfort with people of different backgrounds, abilities, opinions and perceptions and demonstrate the ability to treat all people with equity and respect. Able to work independently and interdependently with other staff and volunteers.
Basic computer skills and knowledge of community resources preferred.
Must have excellent customer service skills and ability to thrive in a dynamic team atmosphere.
Bi-Lingual (Spanish) strongly preferred
Required Education:
High School Diploma
Training / Certification / Licensure:
Certifications per hiring department requirements (i.e.: SERV Safe Certification)
May require a valid Driver's License based on specific hiring program.
