Job Summary:

Serves as the primary contact and/or staff for community screening sites and mobile outreach clinics. The clinician will supervise volunteers and/or students and ensure screenings follow policy and procedures.

Responsibilities:

Essential Job Functions

1. Follows and understands the mission, vision, core values, Employee Standards of Behavior and company policies/procedures.

2. Maintains familiarity with current national standards for recognition developed by the American Diabetes Association and/or the American Association of Diabetes Educators.

3. Ensures appropriate staffing for all community outreach events and performs screening on the mobile unit, as needed. Oversees scheduled site visits to ensure volunteers are properly trained, equipped and quality services delivered. Responsible for staff and volunteer compliance to Mobile Diabetes Center (MDC) standards of care for all screenings and outreach services.

4. Maintains HIPPA compliance during collection of screening data and oversees data collection. Assists with monthly reports analyzing and reporting data, as needed.

5. Participates in quality improvement activities that include but are not limited to participant data, tracking systems and service delivery, as needed.

6. Assists with the maintenance of inventory of MDC supplies and equipment. Request orders and/or maintenance of supplies, as needed.

7. Contacts clients for follow-up and recruitment, as needed.

8. Performs other related duties as assigned by the Chief Student and Community Engagement Officer.

Schedule:

As needed for community-based events. This includes weekends and occasional evenings.

Job Requirements

Education

  • Bachelors of Science in Nursing from an accredited school of nursing required.


Experience

  • Prior patient experience in healthcare preferred.


License/Certifications

  • Current American Heart Association Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.

  • Current valid Registered Nurse (RN) license, valid compact multistate license, or temporary permit while awaiting licensure required.


Skills/Knowledge/Abilities

  • Knowledge of community diabetes resources and community based organizations that serve under-insured and uninsured populations.

  • Awareness and keen appreciation of diverse populations, customs and socioeconomic needs.

  • Knowledge of community based health care outreach and local programs.

  • Demonstrates problem-solving abilities, planning, organizing, time management and interpersonal skills.

  • Ability to proficiently use Microsoft application, computer applications relevant to assigned work and general office equipment.

  • Ability to adhere to all workplace and safety requirements, regulations, standards and practices.

  • Ability to process and manage confidential information with discretion and according to HIPAA protocols.

  • Ability to assign and oversee the work of volunteers and students to ensure customer satisfaction and work productivity.
