As a Home Health Aide (CNA) with Advocate Hospice Services, you’ll provide compassionate, hands-on care that truly matters. You’ll support patients and families during a critical time while working as part of a dedicated Interdisciplinary Team focused on comfort, dignity, and quality of life.

📍 Position Details
- Employment Type: Part-Time (Regular)
- Hours: 24 hours per week
- 3 shifts per week, 8 hours per shift
- Shift: Day Shift
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Service Area: Southwestern Suburbs (travel required)
💙 What You’ll Do
Patient Care & Support
- Deliver safe, respectful, and compassionate care in patients’ homes
- Assist with activities of daily living (ADLs) such as bathing, feeding, positioning, and mobility
- Monitor patient condition and communicate changes to the RN or Case Manager
- Take vital signs as assigned
- Provide personal care, light housekeeping, laundry, and linen changes
- Promote comfort and safety through proper equipment use
- Support hospice and end‑of‑life care with empathy and professionalism
Communication & Teamwork
- Collaborate closely with nurses and interdisciplinary team members
- Communicate clearly with patients, families, and caregivers
- Participate in care discussions and case conferences
- Maintain accurate, timely documentation
- Protect patient privacy in compliance with HIPAA
Quality & Compliance
- Follow all state, federal, and agency guidelines
- Participate in training, education, and quality improvement initiatives
- Cooperate with supervisory visits and scheduling needs
Growth & Flexibility
- Maintain and develop Home Health Aide skills
- Demonstrate flexibility to support patient and team needs
- Take ownership of professional development and learning opportunities
✅ What You’ll Need
- High school diploma or equivalent
- Certified Nursing Assistant (CNA) – State of Illinois
- Minimum of 1 year of nurse’s aide experience
- RN nursing students who meet state requirements may be considered
- Valid driver’s license and reliable transportation
- Strong verbal and written English communication skills
- Ability to care for patients across all age groups
💪 Physical Requirements
- Travel throughout assigned service areas in all weather conditions
- Assist with ambulation, transfers, and repositioning of patients
- Lift up to 35 pounds independently
- Use patient‑handling equipment and assistance for lifts over 35 pounds
- Work in private residences that may have accessibility limitations
