Shift: M-W 8-430 adult social work discharge planning.

Job Summary: The Clinical Social Worker provides skilled Social Work services in areas of comprehensive case management, discharge planning, continuing care services, advocacy, and crisis intervention. This role specializes in handling abuse/neglect cases, crisis intervention and trauma informed care. The Clinical Social Worker conducts comprehensive mental health assessments to determine suicidality including completing the SAFE-T and makes recommendations to the treatment team. The Clinical Social Worker is skilled in safety planning and coordinating wrap around services, and completing legal paperwork (Licensed Mental Health Professional Statement) LMHPS for patients requiring involuntary detention. Minimum Education: Master's degree in Social Work or related behavioral health field. Licensure, Registration and/or Certification: Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC). Work Experience: Minimum 5 years of experience. Hospital or community health agency setting, preferred. Experience with assigned patient populations, preferred. Knowledge, Skills, and Abilities: Knowledge of Microsoft 365 and other applicable software. Excellent communication skills, both written and verbal that present clear and concise information. Effective organization and leadership skills. Ability to collaborate with colleagues of all levels and build cross-functional relationships. Ability to assess, create treatment plans, and execute plans for a full spectrum of mental health conditions. Sound ability to maintain professional and courteous and demonstrate flexibility and adaptability in adynamic work environment. Essential Functions and Responsibilities: Supervises graduate level students and supervises other social workers pursuing their clinical licensure. Receives referrals and assesses in a timely manner those patients and families/significant others who are referred to social service work service. Integrates social work plan into overall patient care through interdisciplinary collaboration. Conducts clinical mental health assessments for patients in behavioral health crisis, legal paperwork for involuntary detention and provides safety planning for patients ready for discharge. Provides skilled Social Work Services in the areas of comprehensive case management, discharge planning, continuing care services, including crisis intervention, abuse and neglect, advanced directives, psychiatric placements/issues, end of life issues, and patient/family education utilizing individual and family modalities. Assesses for Social Determinants of Health (SDOH) and provides needed intervention/resources. Facilitates efforts to obtain insurance coverage for hospital and community-based services. Refers patients, families/significant others, and hospital staff to appropriate services to ensure continuity and quality of care. Understands and utilizes hospital and community-based resources and entitlements including participating in community education and support group activities. Develops and utilizes specialized knowledge of resources related to the needs of specific patient populations. Completes statistical reports, documents the patient care plan, staff interventions/outcomes promptly and completely within the patient's medical record. Collaborates with interdisciplinary team to develop complex discharge plans prospectively and conducts transitional planning for patients moving to nursing homes or other facilities. Decision Making: Independent judgment in making decisions involving non-routine problems under general supervision. Working Relationships: Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
