Job Summary: The VA Eastern Colorado Health Care System, Pharmacy Service is currently recruiting for one (1) Clinical Pharmacy Specialist - Acute Care, at the Rocky Mountain Regional VA Medical Center, in Aurora, Colorado. Major Duties:

  • The Internal Medicine Clinical Pharmacy Specialist (CPS) provides comprehensive medication management and patient care as part of a collaborative team at Rocky Mountain Regional Hospital. Duties include medication reconciliation, rounds, anticoagulation management, pharmacokinetic monitoring, antimicrobial stewardship, teaching, and committee work. Service is provided onsite daily, including weekends and holidays. Major duties and responsibilities of the position include but are not limited to: Customer Service Maintain courteous, respectful, and cooperative relationships with patients, staff, and the public. Respond promptly and professionally to service needs. Demonstrate proactive interpersonal skills to achieve service goals. Work collaboratively as a team member to enhance group productivity and quality. Clinical Direct Patient Care Activities Review patient profiles and participate in daily team rounds. Perform medication reconciliation, pharmacokinetic monitoring, renal dose adjustments, anticoagulant therapy management, and antimicrobial stewardship. Design, implement, and monitor therapeutic drug plans as a mid-level provider. Order, analyze, and review laboratory tests and diagnostic studies necessary for patient monitoring. Follow up on test results to adjust medication therapy, monitoring, and additional testing requirements. Refer patients to physicians when disease progression or adverse drug events require intervention. Clinical Indirect Patient Care Activities Evaluate patient medication regimens for effectiveness, dosing, contraindications, side effects, interactions, and outcomes; communicate findings and alternatives to prescribers. Obtain and document medication histories in medical records. Monitor and report drug errors, adverse reactions, allergies, and compliance issues; document per procedures. Apply knowledge of laboratory values to evaluate patient care and recommend dose adjustments as needed. Review requests for non-formulary or restricted medications for appropriateness and compliance. Assist Pharmacy and Therapeutics committee with medication use evaluations and safety initiatives. Support compliance with national, VISN, and local pharmacy initiatives. Review and verify medication orders for appropriateness and risk reduction. Education and Research Train, in-service, and orient pharmacists, technicians, students, residents, and staff. Utilize effective written and verbal communication in instructional settings. Serve as preceptor for pharmacy students and residents as needed. Assist with new employee orientation sessions. Drug Information Act as an authority on drug usage, interactions, over-dosage, and compliance for staff and students. Utilize up-to-date written and electronic drug information resources. Maintain current knowledge in therapeutics and disease management. Provide timely, accurate responses to drug information inquiries. Educate patients and families on proper medication and device use; assess comprehension and refer to educational programs as necessary. Program Management Document clinical activities and workload in CPRS accurately and promptly, including consults and encounter forms. Participate in medical center and VISN committees as requested. Adhere to all medical center and departmental policies and regulations. Work Schedule: Monday - Friday 6:30 AM - 3 PM, rotating weekends and holidays. Qualifications: Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1)Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Pharmacists assigned to this position must demonstrate the following KSAs:1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.2. Expert understanding of regulatory and quality standards for their program area.3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Post-graduate Year 1 residency and/or Postgraduate Year 2 residency trained or equivalent inpatient experience; Board Certified Pharmacotherapy Specialist (BCPS).
