Clinical Pharmacist - Full Time - Troy, NY
Position Summary:
Clinical Pharmacist must:
- Be proficient in accurately verifying medication orders and dispensing medications:
- Assures safe, effective, and economical use of medications
- Reviews and evaluates medication orders
- Monitors patient medication therapy
- Effectively and appropriately communicates with providers
- Safe storage, preparation, dispensing of medications
- Provides medication counseling for patients and drug information for providers
- Participates in medication event reporting
- Offers leadership to pharmacy technicians and precepts students and/or residents (at least 1 annually) and new hires
- Facilitates inventory control
- Be proficient in providing the following clinical pharmacy services:
- Consult services including medication reconciliation and pain management
- Antimicrobial stewardship
- Transitions of care (i.e., nonformulary medication request review, active participation on rounds)
- Vancomycin and Aminoglycoside monitoring
- Renal dose adjustment monitoring
- IV to PO adjustment monitoring
- Acts as pharmacy lead on a local or regional multidisciplinary or pharmacy specific committee or workgroup
- Contributes to development or review of clinical or operational policies, procedures, protocols OR contribution to the creation/implementation of a new clinical or operational service OR contribution to improvement of existing service
- Provides in-services or presentations to pharmacy staff or other health professionals within SPHP or Trinity Health
- BLS and ACLS are required when providing clinical coverage of intensive care units, emergency departments, step-down units, or cardiac care unit.
Education Requirements:
Doctor of Pharmacy (Pharm.D.)
New York State License and Registration
Experience Requirements:
Excellent interpersonal and organizational skills
Have at least 3 years of pharmacy practice experience within the coverage area
OR one of the following:
- PGY1 residency + 1 year experience
- PGY1 residency + PGY2 residency + 6 months experience
- certification (board certification or certificate program completion that requires renewal preferably in the specialized area of expertise, but BCPS is acceptable)
- Certificate of completion in the area precepted (minimum 14.5 contact hours or equivalent college credit) from an ACPE-accredited certificate program or accredited college/university obtained or renewed in last four years
** Clinical pharmacists who also serve as PGY1 Residency preceptors must meet ASHP Eligibility and Qualification Requirements listed in Standard 4.5 and 4.6 ASHP Accreditation Standards
