Position Title: Case Manager

Classification: SCHADS Crisis Level 2 / Social Level 4
Location: Spring Hill, Brisbane
Employment Type: Full-Time | Maximum Term – 12 Months

We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the Role

As a Case Manager, you will deliver high-quality, person-centred support aligned with national and state strategic plans, local mission plans, and the National Homelessness Stream Model of Care.

You will provide case management services in both community-based and residential settings – specifically in Women's Temporary Supported Accommodation service.

Your practice will be grounded in person-led, recovery-oriented, trauma-informed, and strengths-based approaches, working collaboratively with a multidisciplinary team to deliver services that reflect the vision, mission, and values of The Salvation Army.

How You Will Make an Impact

  • Build strong, respectful relationships that promote choice, self-determination, and active participation
  • Provide effective case management in line with The Salvation Army’s Model of Care and Practice
  • Conduct comprehensive assessments, including needs, risks, and vulnerabilities
  • Undertake ongoing assessment from intake through to exit
  • Develop individualised case plans in collaboration with individuals and families, identifying goals, interventions, and barriers
  • Implement case plans with active participation from service users
  • Support individuals and families to access external services and community supports
  • Advocate on behalf of clients when required to ensure access to appropriate services

What You Will Bring

  • Degree qualification in Social Work, Welfare, Community Development, or a related field preferred or demonstrated high-level sector knowledge and skills gained through relevant experience and professional development
  • Experience in a social service environment, particularly in assessment and case management
  • Experience in the Homelessness Sector – interest in working with women
  • Able to work  shiftwork-  early mornings, nights, weekends and public holidays.
  • Current QLD Working with Children Check (role includes supporting young people and families in residential or home-based settings)
  • Current Driver’s Licence
  • First Aid Certificate (if designated on the roster)
