Job Summary

The Nurse Navigator is a registered nurse who provides individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. This role involves guiding patients through the healthcare continuum, coordinating care, facilitating communication among healthcare providers, and providing education and emotional support throughout the treatment process.

The Navigator will adhere to the American Nurses Association (ANA) standards of professional nursing and performance, and the ANA Code of Ethics to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes. Utilizes the Theory of Human Caring and Caritas in daily practice. "Jean Watson, PhD, RN, AHN-BC, FAAN, Founder / Director, Watson Caring Science Institute."

Essential Functions:

  • Assess the needs of patients and their families and identify potential concerns or barriers to treatment plan. (20%)
  • Serves as a liaison between patient and clinical staff, administration, physicians, managed care companies and community/external resources to coordinate/maximize resources and resolve barriers to plan of care. (20%)
  • Educate patients about their diagnosis, treatment options, procedures, and care plans in collaboration with the multidisciplinary team. (15%)
  • Coordinate appointments, treatments, referrals, and follow-up care with physicians, specialists, and other healthcare providers. (10%)
  • Participate in multidisciplinary team meetings and case conferences to discuss patient care. (10%)
  • Assist patients and families in resolving financial, psycho-social, functional, and administrative issues by advising of options/resources, and intervening as advocate when appropriate. (10%)
  • Address administrative healthcare needs for patients, such as housing, childcare, transportation, prescription costs and finances and provide referrals as appropriate. (10%)
  • Promote positive patient experience. (5%)


Education:

  • Minimum of an Associate's Degree in Nursing required. BSN preferred.


Work Experience:

  • At least one year of experience in a hospital setting required.


Licenses:

  • RN-Registered Nurse - DORA - Department of Regulatory Agencies required.
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross required.


Knowledge, Skills and Abilities:

  • Adherence to Standards of Professional Nursing Practice/ Process / Performance [ANA Standards addendum- signed] and accrediting body standards.
  • Ensures all duties, responsibilities and competencies are conducted in a manner that is effective and appropriate to patients/clients to who care/service is being provided.
  • Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient’s status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided.
  • Employee has completed and met their clinical competency standards
  • Ability to interpret adapt and apply guidelines and procedures
  • Knowledge of perioperative setting in relation to patient outcomes
  • Within scope, require critical thinking skills, decisive judgement, and the ability to work with minimal supervison
  • Ability to communicate clearly, calmly and effectively
  • Microsoft Office Suite required.
  • EPIC documentation preferred.

Schedule: Wednesday afternoon, full day Thursday and Friday. Schedule flexibility may be possible.
