Salisbury Behavioral Health ACT team (Assertive Community Treatment Team) is composed of recovery-focused, multidisciplinary, assertive community treatment teams that provide intensive, individual support in overcoming the barriers to recovery for a person with mental illness. The A.C.T. Team Leader is responsible for supervising person centered behavioral health service delivery (strength-based assessment, identification of goals, planning, implementation, and measurement of outcomes) utilizing the A.C.T.) team model; participating in Performance Improvement (PI) activities, reports areas of concern, participates in the administrative oversight of the program, collects data, supervises and/or implements corrective actions and follow-up methods, ensures staff are accountable to PI activities and utilizes PI findings in job performance evaluations; participates in continuing education programs to promote self-development; implements strategies to achieve goals developed for the program as part of the program’s annual performance improvement plan; insuring the program’s compliance with operating policies and procedures and outside regulatory requirements; representing the program in collaborative meetings with external providers; organizing the work, activities and human resources of the team; directly supervising assigned personnel; communicating with all program personnel; providing periodic management reports.

$1,000 stay BONUS!

  • $250 after 3 months

Full Time Licensed preferred-salary $68K-70K

Full Time Unlicensed- Salary $64K-66K

Required Experience:

  • A minimum of three (3) years of experience in direct care with adults in the following systems: Mental Health, Substance Abuse Treatment, and Vocational Rehabilitation, including two (2) years of community-based treatment.
  • Licensed Preferred; LPC, LCSW, LMFT


Required Education:

  • A Graduate Degree in Psychology, Social Work, Nursing, Rehabilitation, Education 

OR

  • Any Graduate Degree in the field of human services 
  • RN

  • 2 years supervisory experience

Responsibilities/Duties:

  • Works closely with paraprofessional and clinical/professional staff to maintain communication and provide feedback, standardize procedures and expedite PCP implementation.
  • Participates in a first responder on-call system available to consumers and/or his/her natural support network on a 24/7/365 basis; coordinates “first response” resources according to consumer need and the PCP.
  • Provides employees with individual-specific training and training in the knowledge, skills, and abilities required by the population and age to be served.
  • Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and applying corrective action to employees; addressing complaints and resolving problems.
  • Completes Service Authorization Requests as needed and ensure that current service authorization is in place prior to the provision of any service
  • Provides clinical supervision to all employees
  • Minimizes the negative effects of psychiatric symptoms or substance dependence that interfere with the recipient’s daily living and personal development.
  • Completes documentation of services including a daily full service note that includes the name of the person supported, Medicaid identification number, date of service, purpose of contact, describes the interventions, includes the time spent performing the interventions, effectiveness of the intervention, the signature and credentials of the staff providing the service.
  • Other duties as assigned
