Our Veteran Services Homes for Heroes program is a deeply valued initiative within the community. It provides care and support to veterans who are homeless or at risk of homelessness, helping them build sustainable, independent lives in the community of their choice.
 

As a Veteran Services Case Manager within the Homes for Heroes program, you will identify and assess veterans in need, delivering holistic, professional support that meets their welfare and wellbeing needs. Your work will be grounded in high‑quality, client‑centred care.
 

In this role, you will:

  • Assess intake referrals and determine their psychosocial and housing needs in conjunction with the veteran

  • Develop comprehensive transitional support plans for veterans, utilising a veteran-centric, trauma-informed, and recovery-oriented approach

  • Support the veteran to develop adaptive and sustainable strategies to work towards achieving their goals

  • Coordinate community supports (mental health, financial counselling, community and recreational connections) to better manage Veterans mental health and risk factors for homelessness.

  • Monitor, evaluate, and frequently report on the veteran’s progress, ensuring goals are always achievable and sustainable

  • Case manages veterans in the program, building their capacity and daily living skills, and linking them with appropriate support services

  • Work closely in collaboration with community services organisations, homeless and housing specialist service providers, DVA, ex-service organisations (ESOs), and other Government agencies.
     

We need you to have:

  • Experience in Case Management, and supporting a case load of clients with complex support needs

  • Knowledge, understanding, and experience of supporting individuals/veterans in a trauma-informed, recovery-oriented, veteran-centric approach

  • Experience supporting individuals/veterans with complex health needs and linking them with appropriate support agencies

  • Ability to thrive under pressure and handle difficult situations effectively

  • Proven ability to establish and foster positive relationships with internal and external stakeholders.
