Transitions of Care Pharmacist IJob Description:
Clinical Services Management, Development, & Outcomes
Collaborates with physicians and healthcare team.
Communicates effectively and solves problems.
Provides medication therapy consultations and patient education.
Participates in activities to improve patient outcomes (MUE, ADR reporting, drug monitoring, resolving therapy issues).
Handles admission/discharge medication reconciliation.
Ensures safe care transition from inpatient to outpatient.
Supports Meds to Beds Program and outpatient therapy adherence.
Assists with prior authorizations and medication cost issues.
Medication Use & Formulary Management
Assists with formulary management for safe, cost-effective medication use.
Acts as pharmacy specialist for complex cases, adverse drug events, and drug shortages.
Provides verbal and written medication consultations. Education & Training
Stays current with pharmacy trends and shares updates.
Coordinates education, training, and competency assessments.
Precepts students and residents.
Attends conferences and workshops as needed.
Regulatory Compliance
Ensures pharmacy operations comply with policies, regulations, and safety standards.
Demonstrates commitment to Baptist Health values and quality practices.
Requirements: • BS degree and one year Experience required. Kentucky and Indiana Pharmacist License Registration with Kentucky Board of Pharmacy (KY hospitals), within 30 days of hire date if you are a new registrant. If previously registered, you must have registration upon hire.
