The Transitional Housing Coordinator will be on site Monday through Friday. The role of the Housing Coordinator will be to assist individuals in managing household and interpersonal issues, ensure that all house rules are being followed and that the homes are being properly maintained. As well as providing support in conjunction with the individuals team.The Transitional Housing Coordinator will assist in managing household and interpersonal issues, ensuring all house rules are being followed and the home is being properly maintained. As well as providing support in conjunction with the individual teams, in addition to:
Mediate household disagreements or conflicts.
Facilitate monthly house meetings with the individuals in each home.
Assist in coordinating team meetings with each individual and their mental health provider monthly or more often as needed.
Provide direction, advocacy, and assistance in obtaining permanent housing in conjunction with their individual service team.
Monitor and provide support to individuals and/or coordinate with mental health service providers to address housekeeping and hygiene issues.
Coordinate and address home maintenance needs.
Facilitate and track the collection of monthly rent payments.
Observe and report to Lehigh County and the individual service team any suspicion or confirmation of drug or alcohol abuse.
Required Working Conditions
Office - This position takes place in an office setting. While performing the duties of this job, the employee is occasionally exposed to wet and/or humid conditions. The noise level in the work environment is usually moderate and consistent with that of an office environment.
Essential Qualifications/Skills
Must possess strong interpersonal skills to establish and maintain effective working relationships with adults, families, multidisciplinary team members, and other personnel involved in adults’ behavioral program.
Must have knowledge of common behavioral disorders exhibited by adults.
Must have strong written and verbal communication skills.
Must display professional appearance and demeanor.
Essential Physical and Sensory Requirements-With or without the aid of mechanical devices
Light - Must be able to lift a minimum of 30 lbs. Must be able to pull minimum of 15 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand, walk and run. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing, and lifting. Must possess the ability to perform crisis intervention techniques, including physical redirection and guidance of potentially aggressive individuals during crisis situations. Must possess the ability to react quickly and be sufficiently mobile to redirect individuals that may` be injurious to themselves or others. Must be able to physically assist with evacuating individuals during emergency situations.
Position Requirements:
Must have a minimum Associates Degree in related field
Bachelor Degree Preferred
1 to 3 years of experience in the Mental Health Field
