We are looking to hire a Transition Support Coach in Nampa!
A Transition Support Coach functions as a facilitator of interdisciplinary collaboration and care continuity across care settings. Interaction with the patient and caregiver may occur at various touch points across the continuum, based upon assignments, beginning while the patient is in the hospital and extending to include follow up phone calls and potential visits to the patient's discharge environment. This role is focused on provision of information and support for the patient and caregiver in order to ensure effective care transitions, improved self-management skills and enhanced patient-practitioner communication. Some travel is expected.
General Requirements:
Previous experience in Case Management, Care Coordination, home health, and/or managed care setting.
Certification in health coaching preferred.
Education Requirements:
Bachelors Degree in Nursing; Masters in Nursing preferred.
License/Certification Requirements:
Licensed as a Registered Nurse in Idaho. Must obtain license in Oregon within 6 months of hire.
Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire.
Hold a valid Idaho driver’s license.
