Hourly Pay Range:

Position: Transition Care Follow Up Specialist
Location: 4201 Winfield Rd. Warrenville, IL
Full Time 40 hours per week
Hours: Monday-Friday, 8a-5p
Required Travel: No
What you will do:
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
Contacts patients at bedside and or telephonically to schedule outpatient follow up appointments. Requires coordination of patient availability and clinic schedules which may require rescheduling of appointments. Escalates when clinical judgement is needed.
Documents all patient encounters into EMR accurately.
Contacts clinical staff when needed for clarification of order.
Performs general office duties including but not limited to faxing required sensitive information to providers to assist clinical staff with patient services.
Participates in performance improvement activities.

What you will need:
A minimum of six months of healthcare experience And
1 Year Previous Epic User experience
Proficient in medical terminology
Able to handle multiple responsibilities/tasks, stress, and high work load volumes
Ability to easily adapt to change
Strong organizational, written and oral communication skills
Creative problem-solving skills
Ability to demonstrate persistence in follow-up
Superior time management skills, attention to detail, excellent interpersonal skills
Ability to work as part of a multidisciplinary team
