Your Opportunity

We’re thrilled to announce an exciting opportunity to provide vital Case Management support to an impactful program in Coffs Harbour: Tailored Support Coordination Service (TSCS):

Our TSCS program focuses on crafting personalized, goal-oriented support plans for tenants and their families. This service ensures that clients can access essentials services and develop personal goals.

Your role as the Integrated Housing Support Coordinator provides a range of interventions, support coordination and case management services to Mission Australia Housing clients/tenants designed to secure housing services, sustain tenancies, and contribute to improved engagement, wellbeing and independence.

Your key responsibilities will be to:

  • Respond to referrals of clients to the service and complete intake assessments
  • Work with clients to create holistic and strength-based individualised case plans utilising SMART goals including referral to supplementary services as needed.
  • Offer/deliver client focussed interventions to individuals and groups
  • Provide coaching and support to clients to help them sustain their tenancy
  • Liaise with internal and external stakeholders to sustain tenancies.
  • Ensure that all required external and internal data is accurately recorded on a timely basis.

Requirements for Success

  • Tertiary qualification in Community Services, Social Work, Drug and Alcohol, Mental Health or other related discipline preferred
  • Good understanding of the challenges and difficulties in the social housing sector
  • Experience and ability to build rapport with people identifying complex support needs.
  • Established relationships within the service sector preferred.
  • Previous experience developing complex case plans.
  • Established relationships within the service sector preferred.
  • Ability to coordinate, facilitate and participate in group and one-on-one information sessions and activities
  • The ability to work with clients who are facing challenging and multiple barriers to tenancy.
  • Strong verbal and written communication skills
  • Current Driving Licence

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
