Pay Range
Base Salary: $53,000.00 - $66,000.00 (commensurate with experience)
Key Areas of Accountability:
Responds to all inquiries and provides information on VHA’s services, explains VHA services, policies and fee structure.
Provides immediate response to service requests and assignment of Home Service Provider.
Provide crisis intervention to clients waiting for service.
Prepare reports assessing clients initial needs.
Ensure the entry of client referral and intake information into the agency database.
Conducts a financial assessment to determine fee structure and/or refers applicant to appropriate funding source for assessment and subsidy.
Develops a service plan with the client and/or family at the intake level.
Converts in-home service inquiries to VHA clients.
Schedules Child and Family Support Workers (CFSW)and supervises delivery of service to clients. Matches, schedules and supervises CFSW volunteers.
Assesses client needs, matches client to appropriate service provider, sets-up service, records and maintains schedule and all client information on computer.
Linkage clients if deemed ineligible for RCC programs.
Confirms service to clients by telephone, including introduction to VHA, its services and staff.
Supervises service delivery through telephone contact and conducts field supervision.
Provides support for problem solving, working with the Home Service Providers.
Conducts performance evaluations Child and Family Support Workers.
Makes recommendations for training, probation staff, disciplinary action or termination.
Conducts Field Supervision on all clients receiving VHA services
Facilitates team problem solving for service plans, Child & Family Support Workers (HSWs)scheduling, supervision and other operational issues.
Documents and inputs all client intakes, information relating to scheduling, client care plans, changes to service, and all other documentation for client and Family Support Workers (CFSW).
Required Qualifications:
Degree Social Work a minimum of 3 years' experience including client service coordination, supervision and management of Home Service Providers and Personal Support workers.
Experience working with Child and Family Support Workers, Volunteers, and Social Worker students.
Registered with the Ontario College of Social Worker and Social Service Workers.
A valid Ontario Driver's licence and the use of a car.
Knowledge and experience with practical application of employment and health & safety legislation.
Knowledge of the City, TTC routes, and the various components of the Agency preferred.
Excellent interpersonal, communication and organizational skills.
Demonstrates excellent written and verbal communication skills to effectively engage with clients, families, and interdisciplinary teams.
Computer skills in word processing and data entry.
Excellent intake assessment, formulation and case planning skills.
Demonstrated strength in family and individual work and case management.
Knowledge of medical terminology and psychosocial assessment tools.
Demonstrated ability to assess client needs and formulate care plans.
Working knowledge of Mental Health.
Excellent problem-solving skills, solution-focus approach and crisis intervention.
Skilled in service outreach and development.
