Experience supporting Medicaid Home and Community-Based Services (HCBS) waiver programs, including populations such as physically disabled, brain injury, frail/elderly, and intellectually/developmentally disabled (I/DD) preferred.
Prior leadership or supervisory experience in case management or care coordination preferred.
Candidates residing in or near Marion, Reno, Harvey, Kingman, Sedgwick, Harper, or Sumner counties in Kansas are strongly preferred. This role is remote with up to 20% travel.
Position Purpose: Supervises the care coordination team that serves long-term care members to promote quality and efficacy of care management delivery. Supervises day-to-day escalations and care management issues related to members or providers.
