Job Title: Supervisor Caregiver (LPN), ElderOne (Full Time, Days)

Schedule: Days, 8:00am to 4:30pm, one weekend per month.
This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.
SUMMARY:
The Supervisor Caregiver (at ElderOne or CNA at LTC) services supervises the staff to ensure regulatory compliance and quality while maintaining an optimal level of independence for the enrolled frail elderly participants.
All staff members must demonstrate the knowledge and skills to assess, plan care for and provide service to frail elderly participants according to the following responsibilities and standards.
RESPONSIBILITIES
Participates as an active member of the Management Group pursuing and supporting goals and objectives.
Responsible for maintaining weekly employment hourly requirements of caregivers or CNA to ensure
quality coverage for participants/residents.
Works with Human Resources to recruit, interview and hire in a
timely manner.
Discipline, terminate and hire caregivers/CNA in a timely manner as needed.
Responsible for annual coordination of performance appraisal of schedulers and caregivers/CNA’s
Communicates to Long Term Care Supervisors/Nurse Managers events and findings relevant to
participant’s homecare needs that impacts on the team’s decision-making process.
Work with the Multidisciplinary teams to control and prioritize utilization of staffing resources to reflect actual participant homecare needs.
At ElderOne oversee coordination of transitional housing admissions and discharges to ensure quality care
during alternative housing stays.
At ElderOne communicate and coordinate with Community Health Nurses the homecare concerns or issues that may need to be addressed with a home visit or family meeting.
Ensure communication of all HHA changes and time changes as appropriate.
At Long Term Care facilities communicate and coordinate with Nurse Manager, Clinical Leaders and OSD’s concerns or issues that may need to be addressed with a provider, Registered Nurse or family meeting.
Ensure communication of all staff changes and time changes as appropriate.
Coordination of scheduling of staff for mandatory inservices and assist in coordination of staff competencies.
Communicate to Director of Home Care or Director of Nursing unresolved or problematic situations.
Effectively communicate and appropriately document with participants and/or family member issues, concerns or resolution of problems.
Act as a mentor for problem-solving and organization of workload.
Meet regularly with staff to ensure satisfaction with care and serve as liaison with RN or Managers.
Participates in and coordinates appropriate initial orientation of staff.
Ensures staff is oriented to new guidelines and policy and procedure created.
Reviews, updates, and creates new policy and procedure for ensuring adequate competency of all paraprofessionals and improve customer satisfaction. Implements system to ensure inservices and competency requirements are met to ensure compliance with all regulations.
Monitors and approves hours for overtime and unscheduled hours for Staff.
Timely investigation and resolution of concerns/problems and incident reports per policy.
Works with Director of Home Care or Director of Nursing to identify trends in concern/problems to permanently rectify issues.
Participation in the education, orientation and disciplinary actions related to paraprofessional staff.
Monitors and ensures ongoing customer satisfaction via telephone and personal contact.
Meeting with and assistance in resolutions of staff disputes.
Facilitate system revision within the team via Continuous Quality Assurance Program.
Display excellent customer service.
Establish and maintain knowledge of field employee’s and client’s needs and concerns.
Communicate effectively with other multi-disciplinary personnel as necessary.
Work collaboratively with the LPN On-Call coordinator (Weekends).
Serve as a primary liaison with Home Care Plus, ElderOne’s scheduling vendor regarding HHA schedules and vacancies (Weekends).
Determine HHA/CNA reassignment and mandating requirements as needed.
Address all HHA/CNA concerns.
Conduct HHA/CNA rounding.
REQUIRED LICENSURE / CERTIFICATION SKILLS
Graduate from an accredited School of Practical Nursing or equivalent.
Licensed Practical Nurse, current license in the State of New York
1 year experience working with the frail elderly
Strong degree of confidentiality required
PREFERRED QUALIFICATIONS
Ability to communicate effectively
Ability to establish and maintain cooperative working relationships
2-3 years of supervisory experience
