Position Highlights:
Position: Substance Use Disorder (SUD) Navigator
Location: Swedish Hospital
Full Time: Full Time , 40 hours per week
Hours: Days, Monday - Friday
What you will need:
Education: High school diploma. A bachelor’s degree in a related field of study is strongly preferred, but candidates with four years of direct experience in the position’s scope of work will be considered
Minimum Licensure Requirements: Licensure as a Social Worker or Licensed Clinical Social Worker, or a certification listed below, is highly preferred; Certification as a Certified Alcohol and Other Drug Counselor, or Certified Peer Recovery Specialist, Certified Recovery Specialist, or licensure listed above, is strongly preferred.
Minimum Work Experience: 2+ years of in-scope experience required for applicants with relevant education and/or licensure/certifications; 6+ years of directly relevant experience for applicants without a degree or relevant license or certification.
What you will do:
Engages and builds rapport with patients who have recently experienced a medical complication related to substance misuse, or have otherwise presented substance or SUD, with practices of motivational interviewing to identify and encourage recovery-related goals. (20%)
Prior to the patient’s discharge from the emergency department, medical surgical unit, or inpatient program, collaborate with the patient, Endeavor Health colleagues, and external organizations (when appropriate) to craft a care plan for sustained recovery/safety from substance misuse. Work with the attending providers, other clinical departments, pharmacy, and other colleagues in the Endeavor Health system to ensure the care plan is safe and seamless. (20%)
In accordance with the care plan and evolving needs, provides practical assistance in connecting the patient to care/resources across the continuum of substance misuse/SUD-related care, as is needed, appropriate, and respectful of the patient’s willingness to change. The Navigator will be responsible for connecting patients to a wide range of external service providers and coordinating with other healthcare teams within the Endeavor Health system to maximize retention in medications for substance use disorder, other behavioral health services, and coordinated provision of holistic recovery support. (20%)
Routinely assess and monitor the patient's status, needs, and progress by proactively reaching out to the patient/caregiver and ensuring that they are connected to the most appropriate and impactful resources. Regularly follow up with the patient to promote retention in care and to provide continuous recovery supports. (20%)
Provides and/or arranges clinical education including medication management, community resources, financial resources, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making. (5%)
Document healthcare provision in the electronic medical record, perform additional data collection to monitor progress toward outcome goals, and assist in reporting activities to external stakeholders. (15%)
