Position Summary
At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. The Senior Manager, Population Health (End of Life Programs) will primarily lead efforts to build a system for supporting patients as they near end-of-life, such as advance care planning and connecting patients to high-quality palliative care and hospice care when needed.
Core Responsibilities:
Own the execution and the ongoing refinement of select Population Health programs
Work closely with regional leaders to assist with program implementation and execution
Work side by side with other central leaders and teams to ensure effective design and implementation planning including Data, Product, Managed Care Operations, Clinical Operations & Learning, etc.
Accountable to key population health metrics, including quality measures, utilization measures, and financial measures
Other duties as assigned.
Required Qualifications
4+ years of applicable experience in healthcare / population health / working with healthcare providers; or other similar fields
Ability to build the trust and confidence of a wide range of stakeholders
Strong leadership and influencing skills
Incredible level of organization and detail orientation
Analytical and data savvy. Ability to develop business cases, ROI analysis, and financial analysis
Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives
Expert in Excel, Google Apps, project management
Pleasantly persistent approach
Quick learner comfortable with a high level of ambiguity
Education
Bachelor’s degree required
Graduate degree preferred but not required (MBA, MPH)

The typical pay range for this role is:
$46,988.00 - $112,200.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
