Under the direction of the Outpatient Pharmacy Manager, the Specialty Pharmacy Coordinator is responsible for the daily operations of the Specialty Pharmacy according to Salinas Valley Health policies, standards of practice of the profession, and State and Federal laws. Pursuant to the order of a licensed prescriber, the Specialty Pharmacy Coordinator will be responsible for preparing and providing appropriate pharmaceuticals to patients. In this role, Specialty Pharmacy Coordinator will participate in all necessary aspects of the medication-use while providing comprehensive and individualized pharmaceutical care to the patients in their assigned disease state. Pharmaceutical care services include but are not limited to education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.

  • Ensures that the patient’s pharmaceutical needs are effectively met by accurately verifying prescription orders and dispensing pharmaceuticals in a timely manner.

  • Reviews and confirms that prescriptions are written for a legitimate reason under the usual scope of practice of the Prescriber.

  • Ensures the appropriateness of drug, dose, and frequency for each medication ordered before it is dispensed. Clarifies any ambiguities with the Prescriber.

  • Ensure proper storage, handling, and documentation of specialty medications

  • completes critical patient monitoring and reviews patient profile/chart to densify, prevent, or mitigate drug related problems, improper drug or dose selection, sub-therapeutic dosage, overdose, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.

  • Oversees Specialty Pharmacy operations and technician practice/activities.

  • Provides patient education and, as applicable, assists with coordination of care interacting with: designated family members, patient caregivers, social services, and other healthcare providers.

  • Serves as a resource for drug information, including: indications, doses, routes, side effects, adverse reactions, drug interactions, pharmacokinetics, and product availability. Utilizes appropriate reference materials when necessary.

  • Facilitates specialty medication procurement, ordering, and dispensing procedures including but not limited to chemotherapy, controlled substances, etc. as appropriate for assigned specialty disease state.

  • Exercises professional judgment, providing therapeutic and drug information to patients and other health professionals.

  • Makes appropriate evidence-based, patient-centered medication recommendations.

  • Documents clear clinical progress notes to support patient safety oversight related to medication use processes: prescribing, dispensing, patient compliance, and monitoring patient outcomes.

  • Works as an active member of multidisciplinary team and collaborates with healthcare providers.

  • Initiates prior authorization process when required, as well as coordinate the provision of additional patient clinical data or published peer-reviewed articles as necessary to facilitate the prior authorization process through various insurers.

  • Manages prior authorization, insurance approvals, and financial patient assistance programs to facilitate patient access to specialty drugs.

  • Assist in the preparation of the Specialty Pharmacy accreditation and continues with accreditation standards.

  • Provide in-depth counseling and education to patients on medication use, administration techniques, potential side effects and adverse effects, and adherence strategies

  • Recommends therapeutically equivalent medications to Prescribers when appropriate to improve patient access to medications or to assist with lowering patient copay amounts.

  • Participates in the Salinas Valley Health discharge process, streamlining services related to discharge prescriptions.

  • Is a role model for departmental behavior and provides and promotes a positive working environment for the Outpatient Pharmacy staff to ensure strong employee morale, motivation, and productivity.

  • Maintains accuracy and confidentiality of patient Electronic Health Record records.

  • Performs assigned duties to ensure the Specialty Pharmacy’s role and function in the Healthcare System supports the delivery of the highest possible standard of patient care and customer service.

  • Ensures compliance with REMS programs.

  • Participate in quality assurance, regulatory compliance and safety initiatives related to specialty pharmacy operations.

  • Performs other duties as assigned.

Education: PharmD from an ACPE-accredited institution required.

Licensure: Current California Registered Pharmacist license required. Current BLS/Healthcare Provider status per American Heart Association is required. Certified Specialty Pharmacist (CSP) credential from the National Association of Special Pharmacy preferred.

Experience: Two (2) years’ experience in a clinical practice setting with strong working knowledge of pharmacy operations and clinical services is required. Experience in a Specialty Pharmacy setting required. Completion of an ASHP-accredited PGY-1 Pharmacy Residency, or comparable work experience, is
preferred.


Essential Technical/Motor Skills: Ability to operate computer and working knowledge of programs, such as word processing, spreadsheets, and presentations. Proficient with electronic medical records and clinical documentation.

Interpersonal Skills: Ability to relate cooperatively and constructively with patients, families, co-workers, physicians and other staff members. Effective and professional verbal, written, and listening skills. Able to work collaboratively with co-workers and able to take direction. Telephone manners are of a nature that
promotes a positive image of the Department of Pharmacy and encourages teamwork with other professionals.

Pay Range: The hourly rate for this position is $79.87 - $105.00. The range displayed on this job posting reflects the target for new hire salaries for this position.
