Summary :

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.

Job Description:

JOB RESPONSIBILITIES:

Communication and Teamwork: Works collaboratively with team members and utilizes effective communication to provide exceptional patient care. - Assesses patient for therapeutic and clinical interventions and records detail in the patient medical record. - Has proactive involvement and communicates with pharmacy and multidisciplinary teams to improve patient care (e.g. care coordination rounds, multidisciplinary patient care

rounds, staff huddles, etc.).

PROFESSIONALISM: Maintains the expected level of competence and skill in pharmacy practice, exhibits professional behavior, and supports the mission of pharmacy practice excellence and education. - Appropriately addresses conflict and manages difficult situations in a professional manner. - Maintains accountability for managing patients’ medications and medication-related outcomes and providing exceptional patient care. - Maintains and acts on a professional development and continuing education plan. - Maintains competency in their practice area. - Actively participates in the department and health system mission of education and excellence through participation in student, intern and resident preceptorship. - Active membership in professional organizations.

LEADERSHIP: Identifies quality improvement opportunities and provides constructive feedback. Actively collaborates to implement improvements to positively impact patient care. - Directs technicians and pharmacy interns in their daily work through observing their performance, giving feedback, answering questions, providing guidance, and checking the accuracy of their work. - Provides constructive feedback to leaders, peers, technicians and learners. - Represents the pharmacy department on committees, interdisciplinary groups, task forces and/or improvement teams as a pharmacy subject matter expert. - Works on projects and recommends solutions to improve safety or quality of care provided. - Identifies opportunities and recommends solutions to improve the value of care and works with leadership to implement process improvement ideas. - Identifies and recommends pharmacist practice changes. - Demonstrates effective problem-solving skills.

CUSTOMER SERVICE: Provides a high level of customer service to all internal and external customers. Leads service recovery efforts in respective area if needed. - Provides education on medications and medication monitoring to other healthcare providers (formal and informal) as well as to patients, family members, and customers utilizing pharmaceutical expertise. - Actively works to resolve customer concerns and manages difficult situations to promote a high level of customer satisfaction. - Works within the department and individual groups to maintain a focus on common goals and service to the end customer (the patient).

PHARMACY PRACTICE: Independently makes appropriate clinical decisions to provide safe, efficient and evidence-based pharmaceutical care that aligns with departmental and system level medication use policies and procedures. - Follows all departmental and system level policies and procedures related to medication use and ensures regulatory standards are met. - Exhibits fiscal responsibility and leadership within daily clinical

practice through the identification of opportunities to improve revenue cycle enhancement. - Implements evidence-based medicine and follows best practices for patient monitoring and care. - Prioritizes workload to meet patient and other external customer needs. - Implements collaborative practice policies if applicable based on setting. - Utilizes and optimizes technology to efficiently and effectively deliver patient-centered care. - Integrates medication safety practices into all phases of medication use process. Identifies and documents adverse drug events (ADEs), medication errors and near misses. - Assures the use of drugs, technology and clinical protocols meet established best practices, national guidelines and F&MCW standards of care.

QUALIFICATIONS:

  • EXPERIENCE PREFERRED:
    • A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.
  • EDUCATION REQUIRED:
    • Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required.
  • EDUCATION PREFERRED:
    • Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.
  • SPECIAL SKILLS REQUIRED:
    • Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively.
  • SPECIAL SKILLS PREFERRED:
    • Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.
  • LICENSURE REQUIRED:
    • Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
