Liquid Personnel is recruiting experienced Social Workers for its client to support a Hospital Discharge Service with a client based in Newport, working on-site within an acute hospital setting. This service plays a vital role in ensuring patients are supported to leave hospital safely and at the earliest opportunity.
The team operates within a fast-paced, multi-disciplinary environment, working closely with healthcare professionals and partner agencies to deliver effective discharge planning and improve outcomes for individuals and their carers.
The Home First approach is central to the service, offering an exciting opportunity for practitioners to influence both discharge and admission practices while enhancing the overall hospital experience for the local community.
