Description:

Cedar Falls Medical Center / Day Shift / Full-Time
Assesses patients and families to assist in discharge planning and coordinating care in various settings. Adheres to Mercy One's performance expectations and performs all defined services and other related duties in accordance with the Mission, Vision and Values of Mercy One.

What you will Do:   

  • Provide psychosocial and financial assessments and necessary interventions for cancer patients
  • Coordinate appropriate services, resources and referrals
  • Provide emotional support to families and patients
  • Collaborate with other disciplines to meet patient needs.
  •  Provide care appropriate to the age related needs of patient

Hours/Schedule:    

.8 FTE (64 hours biweekly) 

Minimum Qualifications:      

  • Bachelor’s Degree in Social Work
