About the role
Provide social work services to patients, families, and carers within the Transitional Aged Care Program (TACP) in line with service standards
Undertake psychosocial assessments for patients, including identification and assessment of risk to inform care planning and service delivery
Participate in multidisciplinary meetings to support coordinated patient care and safe discharge planning, including referrals to community services and arranging short- and long-term accommodation or placement where required
Provide counselling to clients and their families where required to support adjustment to illness and associated psychosocial challenges
Advocate for patients and carers across health and social service systems, including supporting NCAT Guardianship Division processes where required
Participate in supervision, quality improvement projects, education, research, and student training, while also acting as a resource to other professionals and maintaining accurate clinical documentation, handovers, and statistical records
About you
Current unrestricted NSW Driver’s License Class C or above
Degree in Social Work and eligibility for membership of the Australian Association of Social Workers (AASW) with evidence of clinical experience in a hospital or health related field as a practising Social Worker
Demonstrated clinical skills and knowledge in psychosocial assessment, counselling, therapy interventions, case management, advocacy and discharge planning in a community/rehabilitation/aged care setting
Demonstrated experience and ability to work as a member of multidisciplinary teams in an environment with competing demands, with sound negotiation and liaison skills
Ability to contribute to quality improvement and education activities within the designated specialties and social work services
Demonstrated knowledge of adjustment to aged care illness and disability, solution focused interventions, grief and loss and ability to apply these theories in practice
