Job Summary: James A. Haley Veterans Hospital is looking for Social Worker. This position is located at the James A Haley Veterans' Hospital in Tampa, FL.is a Level I tertiary medical center with general medicine, surgical, psychiatric, rehabilitation, spinal cord injury, nursing home care beds and one of four Polytrauma treatment facilities in the VA, It also encompasses one major outpatient clinic in New Port Richey and three community-based outpatient clinics in Brooksville, Lakeland, and Zephyrhills.

Major Duties:

• The Spinal Cord Injury & Disorder (SCI/D) Inpatient Social Worker will adhere to the NASW Code of Ethics, all local social work policies and/or SOPs and maintain an unrestricted, unsuspended license to practice social work. Adhere to Service Line Agreements, (SLA) Standard Operating Procedures (SOP) and VHA Handbooks for documentation. Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, CARF, and/or revenue sources providing funding to Veterans Health Administration (VHA). Ensure documentation is complete and meets the needs of various interdisciplinary teams involved in Veteran care, including required documentation such as psychosocial assessments within the specified time frames. This includes documentation from the SCI/D program including but not limited to SCI/D Status Notes, SCI/D Emergency Preparedness Notes. Adhere to SLA, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes pre-admission, program participation, and post-discharge follow-up. Independently conducts assessment, provides treatment, education, and develops care plans tailored to the age-specific, psychosocial, and functional needs of the Veteran/Active Duty Service Members, as well as their caregivers, and family. Provide individualize care coordination, case management and discharge planning highlighting patient's strengths, needs, abilities, and preferences within patient interaction, Interdisciplinary Team (IDT) meetings, along with family and collateral contacts. Advise supervisor and IDT on identified treatment goals, priorities, and disposition options and progress. Ensuring patients have access to care via completion of required documents, 1010s, Advanced Directive, and other forms that assist with access to care within the VA or the community. The incumbent will participate in survey accreditation(s) ensuring standards are adhered to within their social work practice, unit/service and at the facility level as well (i.e. infection control and safety). The incumbent will participate in all supervisory-assigned meetings to include individual, team huddles, and will participate in, both social work and matrix, staff meetings. The incumbent will participate on all assigned committees, work groups within or external to the facility. Supervisory approval is required prior to working with a committee within or external to the facility. (See business compliance regarding non-VHA committee participation). The incumbent will participate in coverage for social work services on the assigned unit/service, and when needed for other social work teams inpatient and outpatient as needed. The incumbent will maintain a social work license without interruption. For positions that require use of a government vehicle the incumbent must have and maintain a valid driver's license and pass Occupational Health screening. May provide supervision to a Social Work intern or be generally involved in the MSW training program. Ability to provide consultation services to new social workers, social work graduate students, and other interdisciplinary staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Provides social work services coverage for the assigned unit/service, as well as for additional social work teams as needed or assigned. This includes tasks related to hurricane preparedness and responding to hurricane calls. Other duties as assigned. Work Schedule: Monday through Friday, 7:30 am to 4:00 pm. Qualifications: Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Social Worker, GS-11 Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Full Performance Level] Assignments. This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. Duties include but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills and knowledge to maintain Veteran privacy and confidentiality per policies, handbooks or directives; and acts as an advocate with appropriate VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregiver. Incumbent independently assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. In collaboration with the Veteran, family/caregiver, and interdisciplinary treatment team, the social worker facilitates the delivery of health care services. The social worker identifies family/caregiver stressors, conducts assessment and provides specific interventions. The incumbent provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. The social worker links the Veteran with services, resources, and opportunities, in order to maximize the Veteran's independence, health, and well-being. The social worker conducts timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. The social worker provides interventions independently with Veterans and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems. They provide a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy. They independently formulate and implement a treatment plan including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity. Social workers serve on committees, work groups, and task forces at the facility and VISN level or in the community. They provide subject matter consultation to colleagues and students on the psychosocial treatment of Veterans offering professional opinions based on experience, expertise and role modeling effective social work practice skills.
