Job Summary: The Social Worker is the Coordinator for the Contract Nursing Home (CNH). This includes oversight of all VA paid nursing home care, serves as the Contract Officer Representative (COR) for the Community Nursing Homes Program, functions as the "Subject Matter Expert" for the Community Nursing Home Programs, and administrative and technical responsibility for the development of contracts with nursing homes which provide services for the Community Nursing Home (CNH) program.

Major Duties:

  • Some duties include, but are not limited to: Assuring that the placement of Veterans into the CNH program meets VA administrative and clinical eligibility requirements and works directly with the VAMC Registered Nurse assigned to monitor the clinical care provided to place Veterans. Develops and implements information systems to track a current list of active participants in the program to enable tracking of service activities, utilization of services and other functions. Evaluate program progress and outcomes and utilizes data to continuously modify and improve the program and processes Assures that the annual VA CNH and program reviews are completed as set forth in VA policy and must have a good knowledge of Centers for Medicare/Medicaid standards concerning CNH programs; serves as the Contracting Officer Representative (COR) for the CNH program, assuring that quality assurance related to these programs is within program parameters. Incumbent is responsible for assuring the reporting and investigating of sentinel/adverse events by program staff as necessary, and responds to Congressional Inquiries and completes issue briefs related to provision of care within the CNH program. Assists the VISN 5 Contracting Specialist by meeting the requirements of the COR role in the recruitment and oversite of contracts, ensures that the contracts and agreements adhere to all current VHA Directives, State Licensure Standards and VA-OIG expectations, and conducts intensive review and appraisals of the programs to ensure the accomplishment of objectives and adherence to standards set by governing bodies. Reporting to the CNH Oversight Committee and is responsible for providing the appropriate data to make sound clinical judgments concerning CNH continued status within the program. Provides both informal and formal education to the VA staff, and contracted agencies on any changes in VA policy concerning their specific programs. Recruitment of nursing homes when needed and funding are available. This requires the knowledge and understanding of federal and state standards related to nursing home and non-institutional care programs. Daily processing of CNH consults as the delegated authority to facilitate authorization of care. Investigating all CNH complaints and visiting the CNH facilities as needed to follow up on identified concerns. Completing required monthly tasks to ensure appropriate billing requirements. Qualifications: Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure/Certification: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Differentiating Independent Practice from Advanced Practice Level. Experience must be evaluated to differentiate between independent practice and advanced practice levels. A social worker practicing at the independent level has a generalized knowledge of practice, (which is required for all social workers at the full performance level and above.), whereas the social worker practicing at the advanced level has specialized knowledge of practice typically related to a particular diagnosis or patient population. The advanced practice social worker can be further differentiated from the independent practice social worker by their ability to expand clinical knowledge in the profession, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. A social worker with advanced practice skills and personal qualifications that meet the standard for the senior social worker will have passed an advanced generalist or clinical Association of Social Work Boards (ASWB) examination. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience/Education Requirement: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification Requirement: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. The work is performed in an urgent care clinical setting that exposes the incumbent to the communicable disease common to the diseases that are present within the Emergency Department or Urgent Care Clinic.
