Liquid Personnel is recruiting an Adult Social Worker to join its client’s Older People & Hospital Team, based in Conwy.
What will your responsibilities be?
In this role, you will:
- Hospital Discharge Planning — Lead timely, safe, and person‑centred hospital discharge planning under the Care Act and Discharge to Assess pathways, ensuring assessments are proportionate, strengths‑based, and coordinated with health partners.
- Complex Assessment & Safeguarding — Complete holistic assessments, Mental Capacity Act decisions, and safeguarding enquiries for older adults with complex medical, cognitive, or social needs, ensuring robust risk management and legal compliance.
- Multi‑Agency Coordination — Work collaboratively with ward teams, community health, therapy services, providers, and families to develop effective support plans, prevent delays, and promote independence and wellbeing post‑discharge.
Qualifications & Experience:
To be successful in this role you must have:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
