Position: Social Worker (LSW or LMSW)
Location: Ballard Center, Augusta
Schedule: Part-time (32 hours/week)
Shift: Days (M-F)
What You'll Do:
Provides case management and coordination for patients and/or their families who require a range of educational, social, and advocacy services.
Researches, develops, and delivers educational materials related to patient and family support based on learning styles and needs of populations served.
Assists patients in developing support systems, linking patients and families to community resources as appropriate.
Works collaboratively with staff, payers, and external resources to identify needs, cultivate relationships with internal/external resources, and enhance clinical outcomes.
What You Bring:
Maine state licensure as a Licensed Social Worker (LSW) or Licensed Master Social Worker (LMSW) is required.
Experience working with older adults, families, or individuals with complex medical or social needs.
The ability to collaborate effectively within an interdisciplinary team
Excellent communication and interpersonal skills, including the ability to work with diverse populations
