MaineGeneral Health is seeking a dedicated Social Worker to join our Maine Dartmouth Geriatrics team. This part-time role is 32 hours per week, Monday through Friday, with daytime hours. Job Description:

Position: Social Worker (LSW or LMSW)

Location: Ballard Center, Augusta
Schedule: Part-time (32 hours/week)

Shift:  Days (M-F)

What You'll Do:

  • Provides case management and coordination for patients and/or their families who require a range of educational, social, and advocacy services.

  • Researches, develops, and delivers educational materials related to patient and family support based on learning styles and needs of populations served.

  • Assists patients in developing support systems, linking patients and families to community resources as appropriate.

  • Works collaboratively with staff, payers, and external resources to identify needs, cultivate relationships with internal/external resources, and enhance clinical outcomes.


What You Bring:

  • Maine state licensure as a Licensed Social Worker (LSW) or Licensed Master Social Worker (LMSW) is required.

  • Experience working with older adults, families, or individuals with complex medical or social needs.

  • The ability to collaborate effectively within an interdisciplinary team

  • Excellent communication and interpersonal skills, including the ability to work with diverse populations
